We would love to have you join our team! To join the Hawaii Immunization Coalition, complete the membership form below, and click submit. To pay for or renew your membership ($15 annually) please click on the Pay Now button below, or mail a check to PO Box 25665, Honolulu, HI 96825. HIC membership is based on the calendar year, ending December 31st of each year. Once we receive your membership form and payment, a representative from the coalition will contact you. HIC members receive discounts on registration for workshops and conferences and immunization alerts via email.
